The old calendar allowed two families to hold the same slot. Confirmation was an SMS that sometimes arrived after the appointment. Vaccine week turned the clinic into a queue with a database behind it.
We introduced short-lived locks, a confirmation that actually meant confirmed, and reminders that left the building on time. Peak is the product. Idle Wednesday is not.

We load-tested the Tuesday 8am rush with production-shaped data, not a handful of fake patients. Slot locks expire. Double-book attempts become a clear 'this time just went' instead of a silent overwrite.
No-shows dropped because reminders finally arrived, and because cancelling was a two-tap flow instead of a phone tree.
Staff see the same truth
The desk board and the patient SMS share state. If the family thinks they are booked, the nurse sees the same booking. Split-brain calendars are how clinics lose a morning.
